Appointment Time

Our office is capable of performing all necessary diagnostic tests and treatments in a single visit. Please allow at least 2 hours for a new appointment and 1 hour for a follow-up visit to complete all necessary diagnostic tests and examinations.

What to Bring

  1. Photo ID
  2. Current Insurance card (which will be verified at every visit)
  3. Contact info of your primary care and referring physician
  4. If your insurance requires a physician referral, please obtain the referral prior to your appointment.
  5. Any records or diagnostic tests that have been performed related to your condition
  6. List of medications
  7. Any prescription glasses you may wear or a copy of your eyeglass prescription
  8. Sunglasses as pupillary dilation will make you sensitive to light
  9. New patient forms that you have filled out


Eye Dilation and Transportation

Please remember it is necessary to dilate your pupils with eye drops to have a retinal evaluation. Pupillary dilation makes your vision blurry and your eyes more sensitive to light for several hours. If you are not using public transportation, we recommend considering arranging for someone to drive you home. If you need help with transportation, please contact our office for assistance.

For new patients, please download, complete and bring the following form to your appointment. If you wish to register patient information online, please contact the office with your email address. You will receive an email with instructions on how to set password for the patient portal. The email link will expire in 72 hours or you can use “Forgot Password” link on the login page.

We will verify the specific requirements in your insurance contract regarding deductibles, payments, copayments, and the terms regarding referral numbers, number of allowable visits to specialist, and in-office procedures. If your insurance requires you to pay a co-pay, then you must pay this co-pay at the time of your appointment. Your visit or treatment may be rescheduled if prior authorization from your primary care physician is required. Payment of unauthorized services is then your responsibility. Fees not covered by your insurance company due to unmet deductibles are due as you leave the office. If this should be the case, we offer payment plans to ensure your treatment does not go unfinished.
